At Victorian Nurse Practitioner Services (VNPS), we are guided by clinical excellence, dignity and person-centered care. We provide timely, evidence-based care that enhances safety, wellbeing and quality of life for older adults. Our approach is collaborative and compassionate. We work closely with families, GPs, specialists, aged care providers and community teams to ensure continuity of care and outcomes that reflect what matters most to each person.

Sarah is an Older Adults Nurse Practitioner with extensive experience across acute care, community nursing, aged care, mental health and clinical governance in both the United Kingdom and Australia. She trained as a Registered Nurse in the UK before relocating to Australia in 2007, where she has focused on supporting older people with complex health needs.
Living and working on the Mornington Peninsula for more than 17 years, Sarah recognised the growing need for timely, specialised support for older adults, their families and the clinicians who care for them. Her work centres on delivering responsive, evidence informed, person centred care while strengthening clinical systems and workforce capability.
Her clinical interests include dementia, behaviour support, frailty, mental health in ageing, medication safety, and palliative and end of life care. Alongside practice, she contributes to research examining the impact of Nurse Practitioner led models in residential aged care and works collaboratively with GPs, geriatricians and community services to improve outcomes for older people.

A Nurse Practitioner (NP) is a highly trained Registered Nurse with advanced clinical qualifications. NPs can assess, diagnose, prescribe and manage a range of health conditions.
NPs practice independently and collaborate with GPs, specialists, pharmacies and allied health professionals including physiotherapists, dietitians, OTs and social workers. They work across hospitals, private clinics, aged care, home visits and community services.

Support care across clinics homes, RACFs and community settings
Practice in all states and territories
Are endorsed and regulated by the Nursing and Midwifery Board of Australia (NMBA)
Yes. VNPS can provide on-site NP services to RACFs, including clinical assessment, medication review, restrictive practices, behaviour support, palliative care, training and education and governance support.
Nurse Practitioners do not have access to the same Medicare bulk-billing incentives as GPs. VNPS does not routinely bulk bill. In certain situations, bulk billing will be considered at the NP’s discretion for those who are eligble.
Yes. NPs in Australia are authorised to prescribe a wide range of medications within their scope of practice. All prescribing follows current evidence, safety standards and legislation.
Yes. NPs can refer to medical specialists, allied health practitioners, diagnostic imaging and pathology when required.
Yes. VNPS works in partnership with GPs, geriatricians and other specialists and health care providers.
NP care is designed to complement GP services, improve access, and reduce unnecessary hospital transfers, not to replace GP services.
Yes. VNPS provides clinical assessment, medication review and behaviour support for eligible NDIS participants, depending on need and funding arrangement.
Routine care is provided during business hours. Urgent review may be available for existing partner services and in circumstances such as palliative care delivery. However, VNPS is not an emergency provider.
